There are three plans:

1. Pay out of pocket:

$500 for every well visit

$250 for every sick visit

2. Yearly Plan

Each child 0-2 years old: $1800

Each child 3-6 years old: $1500

Each child 7-11 years old: $1200

Each child 12 years old and older: $1000

Includes:

Sick and well

Routine vaccines

In office testing

Prescription Management.

Claims will be submitted to your insurance and payments sent directly to you.

3. Membership Plan

Per child: $400 per year

Sick visit: $100 all inclusive

Well visit: $150 all inclusive

Telehealth: $50 each
