Please allow up 2-3 weeks for us to determine the status of your application. 

Please make sure your child’s application is completed in its entirety with medical records and insurance card attached.

Do not forget to provide your child/rens DATE OF BIRTH or else we can not process your application in a timely manner.

Dr. Simai has trained our wonderful Physician Assistant Sagee Kahalon to follow Dr. Simai's philosophy in practicing medicine.  Rest assured,  Dr. Simai and Sagee Kahalon constantly collaborate to assure the best care for your child.
